In order to participate in Prescott athletics (including Esports), students and parents need to create and log into their FinalForms account.
Instructions to create and sign in to your FinalForms account:
Sports Physical Form - to be completed by your doctor at the time of the examination.

Prescott School District strives to ensure that all students are able to participate in all activities available to them, regardless of ability to pay. When the District imposes fees it is in compliance with Washington State RCW 28A.325.010.
